Please attach supporting documents related to the request such as topographic map and authorization only if the applicant is not the owner.
Important Notice
This form is not intended for submitting requests for First Water Supply Certificates.
To submit the corresponding application, please make a communication request through the MyEyathPortal with the following information :
- Property address and postal code
- Water meter number and customer number
And attach:
- A copy of your Identity Card/Passport
- A recent ownership verification certificate (E9), showing your Tax Identification Number (ΑΦΜ) and the recent issuance date.
Requests for First Water Supply Certificates submitted through this form will not be processed.
